What is a slipped disc surgery?

Slipped disc surgery, also known as lumbar herniated disc surgery is performed to remove a portion of the disc that is impinging on the nerve root.

Discs sit between the vertebrae, which are the bones of the spine, and are made up of an outer coat of tough fibrous tissue with a jelly-like substance inside. They act as shock-absorbers, cushioning the spine when you jump or run. If the outer part of the disc tears, the cushioning gel can leak out, putting pressure on the nerve which passes between the vertebrae and may cause you pain, generally in the lower back region and down your legs.

The treatment options for a slipped disc, or a lumbar herniated disc will largely depend on the length of time you have had your symptoms and the severity of your back pain. Other considerations include the nature of the symptoms and your age. As a general rule, patients are usually advised to start with 6-12 weeks of non-surgical treatment such as physiotherapy, epidural injections, or medications. There is a wide range of non-surgical treatments available and some treatments may work better for some patients than others.

If after 6-12 weeks, the non-surgical treatment for the slipped disc, or lumbar herniated disc does not provide pain relief and the pain is severe, it is reasonable to consider surgery. Your surgeon may recommend surgery prior to completing a full 6-weeks of non-surgical care if there is severe pain and you are having difficulty maintaining a reasonable level of functioning, or you are experiencing progressive neurological symptoms such as worsening leg weakness and/or numbness.

What happens during a microdiscectomy surgery?

You will have an initial consultation with your surgeon to discuss the suitability of the procedure, to communicate your desired goal, discuss options, and discuss the details of the procedure. Once all of the details are agreed upon and confirmed, a date for the procedure is set.

You will be asked to undergo some tests to confirm diagnosis such as a CT scan, or an MRI scan. You are usually also asked to undergo blood and urine tests to determine your suitability.

Prior to the procedure, a general anaesthetic is usually administered. Once the anaesthetic sets, your will commence the procedure. A small 1 to 1 1/2-inch incision will be made directly over the affected disc. A lighted microscope is used to help your surgeon see the affected area. Your surgeon may remove a small portion of bone that protects the root nerve. With a scissor-like tool, your surgeon will remove the damaged herniated tissue, relieving the pressure on the nerve. The incision is then closed with sutures.

The procedure usually takes between 1-2 hours. Patients typically stay between 1-2 days in the hospital post-procedure for observation. Prior to being discharged, you are often given specific instructions on how to get out of bed properly to avoid stress and strain on your wound site. You may also be taught on how to prevent twisting, flexing, or hyper-extending your back whilst moving around. You are also encouraged to walk, stand, and sit for short periods of time. It is recommended to avoid activities that would strain your spine such as sitting or standing for too long, flexing your spine, bending at the waist, climbing too many stairs, or going for long trips in the car. You should be able to return to work on light duties 6-weeks post-procedure and it is highly recommended to follow a regular exercise program provided by your surgeon or Physiotherapist. This would help to increase your spinal muscle strength and flexibility, and to protect it against future injury.

How do I find the right surgeon?

 Your back pain may not disappear completely. Your mobility will depend largely on your condition prior to the surgery. Another factor that will impact your mobility is your age. Muscle spams are also common after the surgery. Your surgeon will usually prescribe some pain relief and antispasmodic medication for them. 

The success rate for a microdiscectomy is approximately 90-95%. 5-10% of patients may develop a recurrent disc herniation at some point in the future.

 A slipped disc, or a lumbar herniated disc occurs when the circle of connective tissue surrounding the disc breaks down. This allows the soft, gel-like part of the disc to swell and protrude out. It is not always clear what causes the connective tissue to break down, however, there are a number of factors that can cause a slipped disc, or a lumbar herniated disc, such as – traumatic injury to your back, weakness, being overweight, or smoking.
